Extended Medical Benefits

DesLauriers Chiropractic Group Inc. offers direct billing for practice members with extended medical benefits.

In order to direct bill your extended health plan, you will need to provide us with your policy number and identification number upon arriving to your appointment. We will have you sign a consent form authorizing us to direct bill for your care as well as receive payment for your treatment.

Please note: We are only able to bill to one extended health care company per visit. If you have two plans, we can only bill to your primary plan. If you have multiple health care plans, we will collect the remaining outstanding balance and provide you a receipt to submit to your other insurance provider for reimbursement.

How much am I covered for?

We encourage you to review your benefits contract or contact your insurance provider to learn the details of your coverage prior to your appointment. Our office is unable to determine coverage in advance. Insurance eligibility is shared with our office once we submit your claim when you attend your appointment.

Do I need a doctor’s referral?

Chiropractors are a primary care provider and we do not require a doctor’s referral to begin care; however, some insurance plans do require one. Please check with your insurance provider to learn the requirements of your plan.

What if my insurance company only partially covers my visit?

If your insurance company only covers a portion of your visit, we will still submit for you! You will be notified of any outstanding balances and provide you with a receipt upon payment.

Will my insurance cover my user fee portion if I am attending treatments for an open ICBC or WorkSafeBC claim?

If you are attending treatment related to a motor vehicle accident or a work injury, we are unable to direct bill your personal insurance provider for the user fee portion paid for your appointment. We will provide you with a receipt so you can contact your insurance provider regarding reimbursement.

Can you backdate my appointments for insurance purposes?

No. We can only submit to your insurance company on the day of your appointment.
